Who is Steward Health Choice Arizona?

Steward Health Choice Arizona was formed when Steward Health Choice Arizona and Health Choice Integrated Care became one health care plan. Mind and body go together and it’s important to get the right care at the right time. Now you can get your health care from one plan, instead of two. We are a local health plan that cares about keeping you healthy and to help you lead a healthy life.

The joined network of both physical and behavioral health care providers is bigger. Giving you more choices to doctors, pharmacies and counselors near you.

Steward Health Choice Arizona is an AHCCCS Complete Care health plan and starting on October 1, 2018, you can choose us as your plan if you live in:

Central Arizona

  • Maricopa
  • Gila
  • Pinal

Northern Arizona

  • Yavapai
  • Coconino
  • Apache
  • Navajo
  • Mohave

Why did the plans join together?

To make access to health care easier for you with one health plan. Whether you are well, sick or in need of behavioral health services we are here for you. The Arizona Health Care Cost Containment System (AHCCCS) made a new system called “AHCCCS Complete Care” (ACC). This new system joins physical and behavioral health care together. Steward Health Choice Arizona is an ACC plan.

When will changes begin?

Changes will begin on October 1, 2018. Steward Health Choice Arizona and Health Choice Integrated Care will become Steward Health Choice Arizona.

Do all members receive the same services?

All members will receive AHCCCS covered physical and behavioral health services through Steward Health Choice Arizona. Members who are determined SMI, in Apache, Coconino, Mohave, Navajo, Gila and Yavapai counties, will continue to receive services through their existing providers. Health Choice covers physical and behavioral health services for members enrolled in the health plan.

What’s an Integrated Health Home or Behavioral Health Home?

As a Steward Health Choice Arizona member, you will be assigned to an Integrated Health Home or Behavioral Health Home (called “Health Homes”) that is near you, but you can change your PCP or Health Home whenever you like.

An “Integrated Health Home” is a facility in the Steward Health Choice Arizona network that offers both primary care and behavioral health services. Having these services in one place helps you and your primary care provider (PCP) give you whole-health services. Keeping your mind and body healthy!

If you do not live near an Integrated Health Home, you will be assigned to a Behavioral Health Home for all of your behavioral health needs, like counseling or help in a crisis. Your PCP and Behavioral Health Home will work together to make sure you get all the care you need.

Do you cover my prescriptions?
