At Steward Health Choice Arizona we want you to have access to many programs to help you stay healthy.

Check out some of the programs we offer.

Integrated Health Homes

As a Steward Health Choice Arizona member, you will be assigned to an Integrated Health Home or Behavioral Health Home (called “Health Homes”) that is near you, but you can change your PCP or Health Home whenever you like.

An “Integrated Health Home” is a facility in the Steward Health Choice Arizona network that offers both primary care and behavioral health services. Having these services in one place helps you and your primary care provider (PCP) give you whole-health services. Keeping your mind and body healthy!

Stork Program

As an expecting mother, there is a lot you need to know, so we try to make it easy through our Stork Program. This program offers information and guidance from our maternity nurses to help you and your baby stay healthy. We can also help you find community resources for breastfeeding, postpartum depression and HIV testing.

Disease Management

Disease management programs are designed to assist members with developing self-management skills to manage their chronic conditions and live healthy lives. We have a team of experienced nurses, social workers and coordinators, who are trained in each health condition. Our team can assist you with the following conditions (not all inclusive):

  • Diabetes
  • Congestive Heart Failure
  • Hepatitis C

Neonatal Intensive Care (Inpatient Hospitalization NICU) Program

We have dedicated pediatric nurse case managers who assist members admitted to the NICU. The pediatric case managers assist families with transition back to their home and ensure all needed outpatient services are arranged prior to discharge. The pediatric case manager will assist with follow-up appointments, transportation, provide support and education through the transition period.

Transition of Care Program

The Transition of Care Program assists members transitioning from a hospital to home. The program has experienced nurses and coordinators who follow members for thirty (30) days after discharge to ensure all discharge needs are met. We have nurses who also provide face-to-face interactions with members while in the hospital.

Steward Community Connection

Steward Community Connection. A website you can search for free or reduced cost services and programs like food pantries, housing, rides and support groups close to your home. You will also find fitness classes, classes on how to stop smoking, cancer support groups, new mom support groups and MORE!

Just search by your zip code! The website will help you find local programs and services that help you and your family have good health. The programs are near where you live and work.
